It’s not really well known why some people have stronger period pains than others, although a build up of prostaglandin is believed to be responsible. Nobody knows why this happens for some people though.

Painful period also known as dysmenorrhoea is a medical condition due to uterine contractions prior to and during menstruation as a result of some hormonal changes. The reason for severe pains might be due to an underlying disease condition eg endometriosis. Also people respond to pain differently. Women who use certain contraceptive pills do not have much pains as others who do not use as these pills prevent ovulation which preceeds menstrual pain.
